Yanira Serralta, PA-C is one of the health care providers in the Orthonow team. Born in Cuba, she was raised in Finland and speaks English, Spanish, Russian and Finnish. Her caring personality, her ability to be emphatic and attentive listening to her patients, along with her professionalism and trained eye for detecting injuries where many others have missed a diagnosis, make a strong difference in the level of excellent care a patient can receive at their office.

“I got into sports medicine because I have a strong athletic background. That is something that brought up that passion for sports. Working in Orthonow we see head to toe injuries daily, and get a broad experience to treat all kind of injuries. Every single day is different and that is what I enjoy of being part of this medical team. Our main goal is keeping people healthy and helping them recover quickly from their injuries so they can get back to their regular life as fast as possible.”

In the Orthonow office they have many success stories of patients who recover completely, after being in pain for a very long time or barely have any movement due to an injury that was not addressed properly or had been misdiagnosed in other centers. “People appreciate that I have listened to them and taken seriously their concerns and solve their problems.  We do not disregard any symptom. You have to really listen to a patient’s story to find out exactly how to help him feel better” explains Ms. Serralta, “when you see someone’s relief when their symptoms have been addressed, you feel satisfied of having gone the extra mile and listen.”

When a person has a fall and a sprain, tear or fracture is suspected the fear of being handicapped even for a short period of time is an additional situation that the health care provider has to tend to with patience and empathy.

“People gets scared, mostly because they fear of not being able to regain their full function, or of staying in pain later on, or do not know how to function with a cast” says Serralta.” I understand my patient’s fear, and try to address it to help them the best way possible. You have to look at patients as a whole, their circumstances, to understand how important it is the reassurance that if they follow your instructions they can recover quickly and will be able to do again what they used to do.”

“Unfortunately when it comes to injuries, it just happens to an active person, especially children. A child that is active in school, in sports, is prone to accidents. You can’t have them in a bubble.  Being active in the playground, in Physical Education, injuries just happen and sometimes can’t be prevented. It is all about the angle where you hurt the bone.”

Even with the use of helmets, shin guards and other protective gear, people involved in sports can get injuries that need to be tended as soon as possible. “Once an injury happens, has to be addressed immediately, that is the most important thing, not disregarding an injury”, explains Serralta,”because the longer you wait the poorer the outcome and the longer it takes to recover, and the longest you will suffer pain. It is not a matter of prevention but a matter of addressing it faster.”

The whole team of Orthonow share with Serralta her love and enthusiasm for helping people feel better and recover quickly. All patients that visit the office, where no appointment is necessary, are tended to within a very short waiting time and have access to all facilities to provide a complete attention to their recovery needs, being them due to an old injury or a recent accident.

They have a team of doctors in residence specialists in arthroscopy surgery, pain management, sports medicine, upper extremities, hip, knee, foot and ankle, led by Dr. Alejandro Badia, one of the most prestigious upper extremity surgeons in the US.

However, being a specialized urgent care is not synonym of excessive healthcare costs that are commonly associated with tending to medical emergencies. In Orthonow they accept most major medical insurance but also provide affordable fees for those uninsured, with the additional convenience of the location and minimal waiting time for service.

The Orthonow team also followed the CDC guidelines to prevent and timely treat the consequences of concussions in children and athletes by implementing the program “Concussion Baseline Testing”, for children athletes.  These tests asses the brain function of the child by evaluating their learning and memory skills, ability to pay attention or concentrate, problem solving skill, etc.  It is recommended by the CDC for children over 10 years of age that will be involved in sports.

About that program, which is offered for free in Orthonow every Saturday by appointment, Ms. Serralta explains: “Helmets are essential!  But even with protective gear you get people with injuries. Concussions have to be addressed, and they can occur even with a helmet. Their repercussions are very important. With the baseline testing you can have a reference in case a child gets a head injury or concussion.”

“Every parent that is going to have a child engaged in sports should get their baseline testing, because in case of a concussion you check the baseline to ensure everything is well with the child. Parents receive a report and we keep a copy. In case a child has an injury that has to be checked, they have the documentation to compare as reference, for a neurologist or surgeon that might need it.”

Detecting injuries in children

Having an active, happy, healthy child is every parent’s desire. However, sometimes children get injuries or complain of pain that seem to be mild and end up being important or covering major conditions.

Ms. Serralta summarized several aspects that parents have to pay attention in their children to ensure that mild injury on the arm or knee is just a bruise and not something more serious, and when to seek immediate medical attention:

“Head injuries can cause from temporary disturbances in vision or memory, headache, nausea and even changes in personality.” In the event a head injury causes any of the symptoms mentioned before it is a medical emergency that has to be taken to the nearest Emergency Room because it is life threatening.

“There are many injuries that are unique to children. A lot of them we are able to attend and solve in Orthonow and regular urgent care don’t catch them. Children are tricky because they have the growth plates, the growth centers where the bone elongates when the child grows. Fractures sometimes are not obvious. They have to be looked at in a special way. To understand children’s X-rays you fall back to your experience and the clinical exam to rule out major injuries and treat accordingly.”

“Every parent should be aware that when a child is guarding a limb, holding it close to their body and refusing to use it, anything out of the ordinary, pain that lasts more than 2 days in a limb , there must be something wrong with it, it is hurting and must be tended properly. The sooner bone injuries are addressed the better the outcome”.

“If it hurts you have to come and have it checked. It is better to be on the safe side than being in pain for a long time”, concluded Serralta.

Orthonow: Atención de urgencias ortopédicas a la vuelta de la esquina

Un tropiezo en un escalón; una caída de las barras en el parque; un resbalón en la bañera; cualquiera de esas puede ser la causa de un golpe que termine en esguince, fisura o fractura. Los humanos nacemos con 270 huesos que se convierten en 206 al llegar a la edad adulta. Son altas las probabilidades de que alguno de ellos se rompa accidentalmente en el transcurso de la vida.

Los accidentes no suceden de manera planificada y para atender a esas desagradables situaciones de la vida y ayudar a las personas a superarlas exitosamente fue creado el Centro de Atención de Urgencias Ortopédicas Orthonow, ubicado convenientemente en el corazón de Doral. Yanira Serralta, Asistente Medico Certificada, es miembro del equipo de profesionales de la salud que trabajan en Orthonow. Nacida en Cuba, fue criada en Finlandia y habla español, inglés, ruso y finlandés. Su personalidad cálida y amable; su capacidad de empatía y disposición a escuchar atentamente a sus pacientes, aunados a su excelente preparación profesional e intuición para detectar las lesiones que pueden haber sido pasadas por alto para ojos menos entrenados, hacen la diferencia en el excelente nivel de atención y cuidado que los pacientes reciben en su oficina.

“Siempre he sido atlética y estar involucrada en el deporte me llevó a interesarme por la medicina deportiva. Al trabajar en Orthonow he tenido la oportunidad de ver lesiones de los pies a la cabeza. Eso me ha dado una amplia experiencia en el tratamiento de todo tipo de pacientes y situaciones. Cada día es diferente y es lo que más disfruto de trabajar en este equipo. Nuestro objetivo principal es mantener a la gente saludable y ayudarla a recuperar lo más rápido posible de sus lesiones para que puedan retomar su vida normal”.

En Orthonow tienen incontables historias de éxito de pacientes recuperados por completo después de haber pasado mucho tiempo de dolor o de haber perdido movilidad en un miembro por una lesión que no fue tratada a tiempo o diagnosticada erróneamente. “Las personas aprecian mucho ser escuchadas y que sus necesidades sean tomadas en cuenta. Sea un adulto o un niño, una persona que ha tenido una caída y sufre un esguince, fisura o fractura siempre tiene el secreto temor de quedar de algún modo discapacitada, así sea temporalmente. Ese temor y ansiedad deben ser tomados en cuenta también por el médico tratante, con paciencia y empatía.

“Las personas sienten temor de no volver a funcionar al 100% en el miembro afectado; también temen al dolor y se preguntan cómo van a poder vivir así sea temporalmente inmovilizados”, comenta Serralta.”Yo me conecto con el temor de mis pacientes, converso con ellos y trato de comprender lo mejor posible la raíz de esos temores. Tienes que ver al paciente como un todo, comprender sus circunstancias de vida, darles calma y confianza, y asegurarles que si siguen cuidadosamente tus instrucciones muy pronto volverán a su vida normal”.

“Desafortunadamente las personas activas están en riesgo de lesiones, especialmente los niños; un niño activo, que hace deporte, que va al parque tiene siempre alguna probabilidad de lesionarse, no lo puedes meter en una burbuja. Las lesiones accidentales suceden y a veces no pueden prevenirse porque dependen del ángulo en el cual se golpee el hueso”.

Aun con el uso de cascos, coderas, rodilleras y otros protectores, quienes hacen deportes pueden lesionarse y deben atenderse lo antes posible. “Cuando sucede una lesión debe atenderse inmediatamente. Lo más importante es no desestimar una lesión” explica Serralta, “porque cuanto más tarde en atenderse tiene más riesgo de agravarse, la persona tendrá dolor por más tiempo y tardará más en recuperarse. En las lesiones ortopédicas el punto va más allá de la prevención. Lo más importante es la atención inmediata”.

Todo el equipo de Orthonow comparte la mística, amor y entusiasmo de Serralta por ayudar a los pacientes a sentirse bien y recuperarse pronto. Quienes visitan la oficina, con o sin cita, son atendidos con muy poca espera y tienen acceso a todos los recursos para una atención especializada completa para su recuperación, sin importar que la lesión sea nueva o antigua.

El centro cuenta con equipos digitales de Rayos X  y Resonancia Magnética para diagnóstico. Tienen capacidad y material para colocar yesos, cabestrillos y férulas en el sitio y además cuentan con el centro quirúrgico que permite tratar ciertas fracturas abiertas urgentes, reparación de tendones y muchas otras lesiones musculo esqueléticas.

El equipo médico residente, liderado por el eminente cirujano de la mano Dr. Alejandro Badia,  cuenta con especialistas en todas las áreas de la traumatología: cirugía artroscópica, manejo del dolor, medicina deportiva, extremidades superiores, rodilla y cadera, pies y tobillo.

Aun con este equipo tan especializado, los costos en Orthonow no son elevados como sucede en centros generales que atienden todo tipo de urgencias médicas. Siguiendo los lineamientos del CDC para prevenir y tratar oportunamente las consecuencias de las conmoción cerebrales en la cabeza, Orthonow ha implementado el programa “Examen de referencia para Conmociones cerebrales” (Concussion Baseline Testing) para niños atletas. Estos exámenes evalúan las funciones del cerebro, sus capacidades de aprendizaje y memoria, habilidades de concentración y resolución de problemas, etc. El CDC lo recomienda para todos los niños deportistas a partir de los 10 años.

Este programa es ofrecido todos los sábados de manera gratuita en Orthonow, previa cita. La Srta. Serralta explica en que consiste: “Los cascos protectores en la cabeza son esenciales, pero aun con esa protección las personas pueden tener lesiones. Las conmociones cerebrales tienen que atenderse prontamente porque sus repercusiones son sumamente importantes. Con el Examen de referencia si un niño tiene una lesión en la cabeza por un golpe se tiene con que comparar para determinar los efectos de la conmoción cerebral”.

“Recomendamos este examen gratuito a todos los padres que vayan a inscribir a sus hijos en deportes. Los padres reciben una copia del examen un nosotros conservamos otra. Así hay documentación de referencia para un neurólogo o cirujano que pueda requerirla en caso que el niño sufra alguna lesión en la cabeza que provoque conmociones cerebrales”.

Detectando las lesiones en los niños

El sueño de todo padre es tener un hijo activo, saludable y feliz. Cuando un niño sufre una lesión o se queja de algún dolor, aunque sea leve, puede estar cubriendo una condición más importante que es necesario atender con prontitud.

La Srta. Serralta resumió varios aspectos que los padres deben tomar en cuenta y observar en sus hijos, para determinar si ese golpe o lesión que parece leve en el brazo, pierna o rodilla enmascara algo más importante y requiere atención especializada.

“Cualquier lesión en la cabeza que pueda causar fallas en la visión y la memoria, dolor de cabeza, nausea o cambios en la personalidad es una emergencia médica que debe atenderse de inmediato en la sala de emergencia más cercana”.

“Existen sin embargo otras lesiones ortopédicas que solamente ocurren en la infancia. Muchas de ellas las atendemos en Orthonow y en un servicio de urgencia regular no las detectan. Las lesiones ortopédicas infantiles son complejas porque  como ellos tienen las placas de crecimiento, los centros donde los huesos se estiran al crecer, a veces las fracturas no son obvias. Las radiografías infantiles tienen que mirarse de manera especial para comprenderlas y es necesaria mucha experiencia y  un detallado examen clínico para detectar lesiones mayores y tratarlas adecuadamente”.

“Todo padre debe estar pendiente si su hijo trata de protegerse su brazo, pierna, mano, pie; si evita utilizarlo; si se queja de dolor así sea leve por más de dos días. Algo está sucediendo en ese niño y hay que mirarlo para detectar si hay algo más que atender y corregir. Lo más pronto que se atiendan las lesiones de hueso y músculo, más rápida será la recuperación y mejor la prognosis.”

“Si le duele por más de un día, venga para revisarlo. Es mejor atenderse a tiempo que sufrir dolor por más tiempo y además terminar después con un yeso. La recuperación va a ser más larga”, concluye Serralta.
